While I'm not a professional I have played the violin and viola for quite some time; plus my final thesis was about the physics and math of music. The piano is somewhat out of tune indeed in the higher registers. Esquerita seems to like it, because he uses them pretty often. The effect is reinforced due to piano tuning system, which uses equal temperament. This makes all pianos sound out of tune in the highest registers. I suppose Esquerita wanted to create the effect of a bar piano player. Lou Martin did it sometimes on stage during his years with Rory Gallagher, though less extreme.
I came to this conclusion because he almost always sings in tune, including the screams.
